BASIC FUNCTION:

Develop a professional trusting relationship with our patients, organizations and community providers, to ensure coordination of pharmaceutical services and improve outcomes.

The responsibility of the Emergency Department Pharmacist (ED-RPh) is to provide comprehensive Pharmaceutical Care to patients at the time of admission. The primary focus of this pharmacy role will be to obtain a complete and accurate Medication List (in collaboration with the patient) with particular attention to reconciliation of any ambiguous or incomplete medication related information. In addition the ED Pharmacist will be responsible for reviewing, clarifying and processing other medication related orders as time permits. The ED Pharmacist will also serve as a pharmacy liaison and resource for medication related information and therapeutic recommendations for the ED physicians and other professional staff.

EDUCATION AND EXPERIENCE REQUIREMENTS:

The Emergency Department Pharmacist must have a Bachelor of Pharmacy or Doctor of Pharmacy (Pharm.D.) degree (preferred but not required) from an accredited college of pharmacy. He/She must be licensed to practice pharmacy in New York State or must be eligible for New York State licensure. Ideally he/she should have completed a clinical pharmacy residency, fellowship or obtained equivalent inpatient hospital experience.

PRIMARY RESPONSIBILITIES OF THE POSITION:

Comply with all state and federal laws and regulations, policies and procedures of Ellis Medicine and standards of affiliated accrediting organizations.

Must be able to demonstrate a complete understanding and knowledge of New York State and Federal Laws governing the practice of pharmacy. New York State Licensure must be maintained in good standing including all continued educational requirements.

Maintains current knowledge and skills necessary to perform competently. Work must be accurate, thorough and attention to detail is evident.

Assure that company resources are utilized responsibly and that the working environment is clean, safe and in compliance with required workplace standards.

Communicate relevant departmental information to the nursing and medical staff as needed.

Seek opportunities in collaboration with management to streamline processes, implement process improvement activities and resolve conflicts.

All other duties as assigned

The ED-RPh will focus on all patients being admitted to an inpatient unit. This focus will include but not limited to: Review of medical history with primary attention to admission medication reconciliation, emergency department medication orders and subsequent admission medication orders.

Primary attention to high risk patients, patients on more than 5 medications, high risk and high-cost medications.

Collect and aggregate data in a manner that allows presentation in a clear and concise format and as required by all collaborating providers.

Act as a liaison between the medical, nursing and pharmacy staffs in addressing questions, concerns or issues as necessary and related to ED medication workflow.

Coordinate with Ellis Nott Street Pharmacy or patient’s pharmacy of choice, to ensure any discharge medications are filled at the time of discharge from the ED if appropriate.
