Bilingual(Spanish) Nurse Practitioner – Traveling - Assessments – California

Advantmed is hiring motivated Nurse Practitioner's to conduct health assessments with members in their homes. This is an incredible opportunity to join our fast-growing healthcare tech company offering you unique schedule flexibility.

Responsibilities:

· Perform annual wellness visits and health assessments on a population with chronic conditions

· Provide patient health education opportunities

· Assist in closing quality care gaps (i.e. screenings and labs)

· An enthusiastic collaborator contributing to the enhancement of care delivery

Qualifications:

· Must have a valid unencumbered NP License for California

· Previous In-home Risk Assessment experience a plus but not required

· 3 years patient care experience required. (Experience in primary care/adult/geriatric, EMR skills preferred)

· Bi-lingual a plus!

· Competitive wages (Position is a contractor position based on client visits)

*Travel will consist of your home area and the surrounding areas

Job Types: Full-time, Temporary, Contract, Part-time

Pay: $100.00 - $110.00 per hour
