JOB SUMMARY:
- The Hospice Licensed Clinical Hospice Social Worker helps clients, and their families navigate planning for end-of-life care; understand their treatment plan and be vocal about their needs; manage the stressed of debilitating physical illnesses, including emotional, familial, and financial; overcome crisis situations; and connect to other support services. They are advocates for patients and their families and know what their clients need and what resources are available within and outside of the hospice setting to help them.
The Palliative Care Licensed Clinical Social Worker addresses clients physical, mental, social, and spiritual well-being in all disease stages, and accompany the client from diagnosis to cure, or from diagnosis to hospice. Palliative Social Workers conduct home visits to help clients and their families establish effective palliative care plans at home, including educating and initiating advanced care planning, community resources, providing psychotherapy and counseling services as needed, and assisting with resources to identify and support the client to meet their identified goals of care. They also may act as an advocate with other medical providers.
OUR MISSION:
- Master of Social Work (MSW) degree from an institution accredited by the Counsel on Social Work Education
- MUST have LCSW licensure
- Minimum of one (1) years’ experience in a healthcare setting preferred
- Valid driver’s license and evidence of insurance coverage
