JOB TITLE: PHYSICAL THERAPIST

DEPARTMENT: Clinical

MAIN FUNCTION: Organizes and conducts medically prescribed physical therapy programs in the home to restore function and prevent disability following disease, injury, or loss of a body part.

Collaborates with the interdisciplinary team to maximize patients' physical outcomes. Participates in the development of the physical therapy program.

SUPERVISED BY: Clinical Supervisor

SUPERVISES DIRECTLY: HHA, Physical Therapy Assistant

LICENSURE/CERTIFICATION: Licensed in the state where employed. CPR preferred.

FLSA STATUS: Exempt

EDUCATION/EXPERIENCE: Graduate of an approved physical therapy program. Minimum of one year of physical therapy experience.

PROFESSIONAL QUALIFICATIONS: Strong clinical assessment, patient teaching, and organizational skills.

WORKING CONDITIONS: Exposed to the risk of disease and bloodborne pathogens. Extensive car travel.

Must be able to adapt to difficult working situations i.e. unsanitary homes, and potentially unsafe areas.

PHYSICAL REQUIREMENTS:

Frequent bending, pulling, stooping, kneeling, reaching, and twisting.

May be required to lift, push or roll 50lbs.

Frequent heavy physical exertion may be required inpatient transfers and lifting.

Frequently assisting patients with ambulation exercises.

DUTIES:

• Performs physical therapy procedures, evaluations, and diagnostic tests on patients who have been medically refined.
• Teaches appropriate treatment procedures and supportive activities to patients and family members.
• Develops individualized patient plans for modifying equipment appliances and adapting the physical surroundings in the patient's home to meet their altered ambulation needs.
• Serves as a consultant to client home health agencies for questions pertaining to Physical Therapy. Participates in policy development related to Physical Therapy activities.

20. Upon recognition of a problem/opportunity thoroughly investigates and analyzes relevant facts, and using good judgment, determines the appropriate decision.
