Type Of Work

The position will be based at our Lakeview, Wicker Park, and West Loop clinics, with limited opportunities for remote access via telemedicine (please do not apply if you are only available for telemedicine)

A significant work assignment for this position involves in-person evaluation of patients presenting for COVID testing.

We see primarily patients with an age range between 21-65. Our focus is helping effect lifestyle changes and preventing disease processes to begin in the first place. We also have a full-time naturopathy physician and a nutritionist who work closely with our patients.

Requirements

A willingness to evaluate patients presenting for COVID testing

Weekend, in addition to weekday availability (closed all holidays)

Preferred

Primary/urgent care experience

Mental health experience

LICENSES AND CERTIFICATIONS REQUIRED:

• Licensed to practice in the State of Illinois
• Current DEA number and prescriptive authority
• Current state license and the State of Illinois controlled substance
• Maintains membership in state, national and local professional organizations that enhance professional practice and personal growth
• Cardiopulmonary resuscitation (CPR) certification

TRAINING AND EXPERIENCE:
1. 1-2 years minimum experience working in a primary/urgent care clinic environment

2. Experience caring for patients of all ages

3. Mental health experience is a plus.

4. Experience in CPT/ICD coding
