​Reports to: Clinical Director

Brief Position Summary:
Provides health care to patients an office setting. Works in consultation, cooperation and coordination through a collaborative physician. Functions according to the established standards and practices of medicine for Nurse Practitioners in compliance with New York State regulations.

Essential Functions:

• Assess health conditions and collect patient data through use of interviews, observation and elimination.
• Identifies the need for additional data collection including the need for data collection from other health providers;
• Documents findings and actions appropriately in the patient's medical records
• Assists in educating the patient and family regarding medical conditions, procedures, and medications.
• Educates the patient on the importance of preventative as necessary and appropriate.
• Recommends community resources to meet patient and family needs.
• Triage patient's phone calls and provide consultations/recommendations for appropriate treatment
• Performs evaluations and treatment procedures in emergency situations when necessary.
• Maintains patient confidentiality and adheres to standard HIPPA regulations.
• Attends meetings and participates accordingly.
• Maintains CMEs
• Performs related work as required or assigned.
• Order laboratory testing, preform PAP-smears (with Chaperone), order radiology testing such as mammograms, ultrasounds, PFTs and make referrals to medical specialists when necessary
• Maintain a professional demeanor and work environment

• Demonstrates a strong commitment to practice and values of the practice
• Must continually demonstrate safety procedure for the clinic and utilize universal precautions when appropriate.
• Must display an image of person and professional competence (including attire and hygiene practices)
• My display knowledge and adherence to the philosophy of Continuous Quality Improvement

Qualifications
    -Education: Master's Degree in Nursing and Primary Health Care are required.

    -Certifications: Current certifications in BLS, CPR, ACLS are required with current RN License and Nurse Practitioner licensure from the American Nursing Credentialing Center (ANCC)

Pay: $140,000.00 - $200,000.00 per year
